What You Will Be Doing

Peninsula Regional Assessment Service (RAS) is seeking to employ an enthusiastic and motivated Home Support Assessor to complement our existing multi-disciplinary team at Mt. Eliza Aged Care Assessment Service (MEACAS).

Practicing within the framework of My Aged Care and within Peninsula Health organisational requirements, the Home Support Assessor will provide a thorough and independent entry level assessment of the aged client (and some younger adults when appropriate), and with the client develop care plans that promote the restoration of function, Re-ablement and/or maximise independence. The Home Support Assessor will formulate and review support plans and facilitate referrals for Commonwealth Home Support (CHSP) services to enable older people to engage the range of community resources in order to meet their specific needs.

Key Responsibilities

The Peninsula Regional Assessment Service (RAS) is delivered across the boundaries of the Frankston Council Local Government Area (LGA), therefore driving significant distances is a requirement for the position. The successful candidate will possess proven clinical assessment skills and excellent and functional computer literacy; using a laptop or other electronic device in the community environment is an essential requirement of the position. Together with a commitment to delivering a first class service to the aged client group, Home Support Assessors are required to develop and maintain essential working relationships with community providers and associated stakeholders.

The successful candidate will demonstrate an ability to work autonomously as an effective member of the team, with commitment to the delivery of coordinated and innovative practice within the community aged care service. You will be dedicated to best practice principles and be active towards achieving personal development and team progression. You will be required to work across the Frankston LGA to provide sustainable, well-managed and efficient service delivery, together with contributing to the development of strategies that improve Aged Care Assessment Service. This includes an ability to adapt and operate within a changing service environment.

The position is available on a Fixed Term Full Time basis; previous community health, assessment or aged care experience are therefore highly desirable.

Essential Criteria

The successful candidate will hold a Diploma in Welfare Studies OR Diploma in Nursing or Diploma in Community Services.

• Computer literacy
• Commitment to delivering a first class service to the aged client group
• Develop and maintain essential working relationships with community providers and associated stakeholders
• Registration with AHPRA as required
