About Peninsula Health Peninsula Health provides public healthcare services to residents of the Mornington Peninsula.
Our hospitals and emergency departments are located at Frankston and Rosebud, and we provide a wide range of community health, dental, rehabilitation, mental health, palliative care ad aged care services from campuses at Frankston, Hastings, Mornington and Rosebud.

Position Summary Peninsula Health (PH) currently has an opportunity for an experienced and person centred clinician to join our ACCESS team on a fixed term part time basis. Collaborating closely with a range of services and teams, the registered nurse/ health professional will provide information, advice and referrals to a range of Peninsula Health community, rehabilitation and specialist services ACCESS Workers contribute to the provision of an integrated intake and referral service providing information and assistance to service providers and consumer/ community enquiries
Key Responsibilities • Develop a strong knowledge of services available both within Peninsula Health and within the community sector and provide relevant information to consumers. • Complete initial safety screening, provide information and referral to services in accordance with funding guidelines and established service guides. • Conduct initial needs identification with the specific purpose of clarifying the clients’ needs, eligibility and priority for all service referrals. • Develop and maintain an up-to-date knowledge of eligibility and admission criteria and waiting list times for Peninsula Health services to ensure the best service to meet consumer needs is identified.
