Position Summary

Peninsula Health Mental Health Service are seeking innovative energetic and experienced Community Mental Health Clinicians to be part of the Access and Assessment Team (AAT) incorporating Mental Health Telephone Triage (MHTT) and Police Ambulance and Clinician Early Response (PACER).

The AAT is a community based initial comprehensive assessment service that responds in a timely manner according to the state-wide mental health triage scales for Victorian mental health services. The AAT conducts face-to-face holistic assessments in the Frankston, Mornington or Rosebud community clinics and/or the client’s home, providing subsequent referral and integrated planning strategies to both PHMHS service programs and external service providers and includes the needs of the carer/family supporting the client. The AAT also has the capacity to provide brief time limited intervention treatment services with a particular focus on suicide prevention and deliberate self-harm.

The Mental Health Telephone Triage service provides comprehensive and timely telephone screening, triage and review of the mental health status of clients, with the aim of categorising and co-ordinating referrals to the most appropriate assessment and treatment option within either the Peninsula Health Mental Health Service or through an external service provider.

As a member of this multidisciplinary team, your skills will be utilised within both teams; to provide comprehensive face to face assessments or timely telephone screening and triage and review of mental health status of clients.

Key Responsibilities

• Positions at this level utilise advanced clinical knowledge and skills to provide quality mental health services within their professional scope of practice.
• Assess, plan, implement, evaluate and document comprehensive care within the area in accordance with evidence based best practice standards and Peninsula Health Values.
• Ensure the safety of the public, patients and staff in relation to the provision of a safe environment from both OH&S and Infection Control Perspectives.
• Complete all mandatory training as per policy and participate in organised training and development activities.
• Assist in the orientation of new staff and the supervision and instruction of students.
• Undertake mental, social and physical assessments that lead to recommendations for treatment.
• Prepare care plans in collaboration with clients and their nominated support person(s).
• Provide education regarding prescribed medications and administer (nursing) and assess effects of same.
• Provide counselling and therapy to clients.
• Be responsible for maintaining state or the art knowledge of the mental health speciality.
• Contribute to the training and development of direct care staff regarding the speciality area.
• Liaise with internal and external service providers as required.

Essential Criteria

• Qualified and Registered with relevant National Board
• Post Graduate qualification in Mental Health Nursing or a specialist undergraduate Mental Health nursing program
• Demonstrated clinical knowledge and skills commensurate with experience
• Commitment to delivering high quality outcomes for consumers
• Proven ability in the management of clients with a major mental illness and in the techniques of assessment, crisis intervention, counselling and psychosocial rehabilitation
• Demonstrated working knowledge and understanding of the Victorian Mental Health Act 2014, the National Standards for Mental Health Services 2010, the National Standards for the Mental Health Workforce 2013 and the Australian Council on Healthcare Standards including EQuIP
• Able to use basic computer software applications and prepares written documentation using clear, concise and grammatically correct language

Selection Criteria Essential: Qualified and Registered, with relevant National Board (Clinical psychology for Psychologist; Mental Health nursing qualification such as a post graduate qualification in psychiatric/mental health nursing or a specialist undergraduate psychiatric/mental health nursing program).
Essential: Demonstrated clinical knowledge and skills commensurate with experience, including extensive and recent experience in the mental health field including acute inpatient services and/or community mental health care.
Essential: Takes responsibility for own actions and is proactive and self-starting. Prepares written documentation using clear, concise and grammatically correct language. Able to use basic computer software applications.
Essential: Demonstrated working knowledge and understanding of the Victorian Mental Health Act 2014, the National Standards for Mental Health Services 2010, the National Standards for the Mental Health Workforce 2002 and the Australian Council on Healthcare Standards including EQuIP.
