What You Will Be Doing

The Mt. Eliza Assessment Service is seeking to employ an enthusiastic and motivated Comprehensive Assessor to complement the existing multi-disciplinary team. Practicing within the framework of My Aged Care and within Peninsula Health organisational requirements, the Comprehensive Assessor will provide a comprehensive and independent assessment of the aged client (and some younger adults when appropriate), and with the client develop care plans that promote the restoration of function and/or maximise independence. The Comprehensive Assessor will also review support plans and facilitate referrals or provide service coordination for older people to engage the range of community or hospital based services in order to meet their specific needs.

Key Responsibilities

The Mt. Eliza Aged Care Assessment Service is delivered across the boundaries of Frankston, Mornington Peninsula, City of Casey and Cardinia, therefore driving significant distances is a requirement for the position. The successful candidate will possess proven clinical assessment skills and excellent and functional computer literacy; using a laptop or other electronic device in the community environment is an essential requirement of the position. Together with a commitment to delivering a first class service to the aged client group, Comprehensive Assessors are required to develop and maintain essential working relationships with community providers and associated stakeholders.

The position is available on a Fixed Term Part Time basis: previous ACAS experience is therefore highly desirable. The successful candidate will hold formal qualifications in either Allied Health or Nursing at a minimum of graduate level.

Selection Criteria Essential: Do you have formal Qualifications in either Allied Health (Physiotherapy, Occupational Therapy, Social Work) or Nursing?
Essential: Do you have a current, valid, unsuspended Victorian Driver's License?
