What You Will Be Doing

The Care Coordination Team Leader role sits within Peninsula Health’s Community Health Independence Program and is based on the Wagner Chronic Care Model, an evidenced based systems framework. The Team Leader is responsible for leading a multi-disciplinary team to the delivery of Care Coordination and allied health intervention within their scope of practice, to clients who have chronic health conditions and/or complex healthcare needs.

What You Need

We anticipate you will have:
• Allied Health Practitioner with current registration with Australian Health Practitioner Regulation Agency (AHPRA) or relevant professional organisation (if Social Work).
•Demonstrated ability to conduct comprehensive person centered assessments and determine the most appropriate intervention within professional scope of practice.
•Demonstrated leadership experience in working both independently and as a part of a multi- disciplinary team, with the ability to work effectively, supportively and flexibly.
•Demonstrated experience in providing clinical supervision.
•Demonstrated ability to manage multiple competing tasks and effectively prioritise.
•Demonstrated commitment to professional development and evidence based practice
•Demonstrated commitment to quality improvement activities.
•Specialist experience and knowledge regarding chronic disease management.
•Demonstrated experience working with at-risk and vulnerable people with complex needs.
•Excellent written, verbal, interpersonal and computer literacy and communication skills
•Ability to supervise undergraduate students.
•Current Victorian Driver’s Licence.
