What You Will Be Doing

This position sits within Peninsula Health’s (PH) AOD Program and is based in the Community health sites, including Frankston, Hastings, and Rosebud. The position involves supporting complex AOD client’s throughout their treatment journey using a care coordination approach. It involves providing individualised and flexible support that supplements clients’ AOD treatment, and liaising with other key staff and services that are involved in the clients care. The role also involves facilitating referring on to appropriate internal and external services and programs.

What You Need

We anticipate you will have:
• Certificate IV, or Diploma in Alcohol and other Drugs or the Core Competencies
• Demonstrated experience working with at-risk and vulnerable people with dual diagnosis
• Demonstrated experience in working both independently and as a part of a multi- disciplinary team, with the ability to work effectively, supportively and flexibly
• Demonstrated knowledge in providing care coordination or case management to complex and high risk clients
• Current Victorian drivers licence.

Selection Criteria Essential: • Qualification appropriate to the position such as Certificate IV, or Diploma in Alcohol and other Drugs or Postgraduate course in Addiction
Essential: • Demonstrated experience working with at-risk and vulnerable people with dual diagnosis.
Essential: • Current Victorian drivers licence.
