What You Will Be Doing
The Community Care Occupational Therapist forms part of Peninsula Health's Health Independence Program and is based on the Wagner Chronic Care Model, an evidenced based systems framework.
The Community Care OT is responsible for coordinating intensive short term (3-6 months) outreach care and delivering allied health interventions within their clinical scope of practice to clients who have chronic health conditions and/or complex healthcare needs.
The role involves facilitating collaborative integrated care planning across acute and community based services. The role of the Community Care OT provides occupational therapy interventions, supports both professionals and service users to navigate the health service system, ensuring responsive, patient-centric outcomes for all.
What You Need
· Current registration with Australian Health Practitioner Regulation Agency (AHPRA) or relevant professional organisation. · Demonstrated Occupational Therapy experienced in providing comprehensive OT assessment in home setting and provision of equipment needs. · Specialist experience and knowledge regarding chronic disease management. · Demonstrated experience working with at-risk and vulnerable people with complex needs. · Current Victorian Driver's Licence. · Well developed written, verbal, interpersonal and computer literacy and communication skills. · Demonstrated experience in working both independently and as a part of a multi-disciplinary team, with the ability to work effectively, supportively and flexibly. · Demonstrated ability to manage multiple competing tasks and effectively prioritise. · Demonstrated commitment to professional development and evidence based practice. · Demonstrated commitment to quality improvement activities. · Demonstrated ability to supervise Occupational Therapy students.
