1.0EFT (40 hours p/f – 3 days TCP and 2 days FCRP) Limited Term Position (12 months)
Position Summary Peninsula health currently has an opportunity for an experienced Grade 2 Dietitian to work across both the Community Rehabilitation Program (CRP), and the Transition Care Program (TCP). The Dietitian will work within dynamic multidisciplinary teams, providing excellent assessment, management plans and discharge planning – with the emphasis on achieving optimal outcomes for the clients – all related to the client specific goals.
Key Responsibilities • Provide quality, safe and cost effective Dietetic services as part of a multidisciplinary team, within a Centre/Facility based and Home/Community based models of care. • Build and expand professional skill levels to ensure that clients receive the best possible service reflecting up to date evidence based practice. • Work closely with all CRP/TCP team members. • Provide clinical supervision and leadership to Grade 1 staff, allied health assistants and students if required. • Demonstrate positive workplace behaviors and relationships at all times.
Essential Criteria • Current Victorian Driver’s license • Current Accredited Practising Dietitian • Ability to competently perform high quality Dietetic assessment, treatment and patient management, including thorough and appropriate discharge planning • Demonstrate a high level of experience and competence in rehabilitation and aged care • Demonstrated experience in the supervision of Dietetic staff and students • Well-developed interpersonal and communication skills • Demonstrated ability and experience working within a multidisciplinary team • Demonstrated commitment to continuous and innovative improvement through quality and research activities
