Position Summary

The Sub-Acute Assessment Service (SAAS) is responsible for the assessment of patients at Frankston and Rosebud Hospitals, where inpatient rehabilitation or geriatric evaluation and management may be indicated. It also manages coordination of admissions to Peninsula Health’s 180 sub-acute inpatient beds, from within Peninsula health as well as from external health services and community based provider referrals. The service aims to improve responsiveness and enhance the capacity to assess and transfer patients in a timely way from acute beds throughout Peninsula Health including Emergency Sort Stay Unit (ESSU) and where appropriate, the Emergency Department, to Sub-Acute Services.

The SAAS Team leader is responsible for managing the SAAS Nurses workload and maintaining flow across Peninsula Health. The Team Leader will clinically assess patients as well as lead the team of SAAS RN`s. The SAAS Nurses role is to assess patients in Peninsula health Acute Wards, and occasionally external private hospitals, for their suitability for transfer to a sub-acute setting, including identification of goals for their sub-acute stay, ability to participate in sub-acute programs, and medical stability. The role works in conjunction with Consultants in Rehabilitation and Aged Care Medicine, and Sub-Acute Assessment Service Admissions Coordinators. It also consults with treating teams and other clinical staff within Peninsula health, developing positive and fruitful relationships supporting appropriate discharge planning.

The SAAS Team Leader will provide expert advice and support SAAS nurses staff in acute settings in relation to the care of frail aged patients with special needs as a consequence of their multiple comorbidities.

Key Responsibilities

• In addition to the key responsibilities of the SAAS Nurse the SAAS Team Leader is required to :
• Clinically assess patients as per SAAS RN PD
• Responsible for setting the departmental budget for the financial year in conjunction with the Operations Director
• Responsible for effectively managing the budget to ensure budget is achieved
• Monitor eft in conjunction with the Operations Director
• Provide clinical and education leadership to the group
• Responsible for performance management of all staff in the team’
• Manage bed allocation to optimise patient flow and care with sub-acute sites by communicating with sub-acute wards, Acute Bed Manager and acute PSM’s at both Frankston and Rosebud Hospital sites.
• Manage the SAAS roster as per Peninsula Health Rostering Guidelines and EBA
• Ensure SAAS Nurses are up to date with Mandatory Training:
• Manage incidents, complaints and staff issues as they occur and escalate where appropriate.
• Lead and manage the team to achieve Unit/Department Key Performance Indicators.
• Promote a culture of continuous service improvement within the Unit/Department by monitoring standards of service and practice.
• Manage accreditation requirements for the service
• Attend relevant committee meetings as required
• Manage, monitor all workcover staff and their return to work plans
• Manage and monitor all policy and procedure requirements
• Effectively manage portfolios ensuring organisational requirements are met
• Any other Any other tasks as directed by the Operations Director or delegate

Essential Criteria

• Registered Nurse with current AHPRA registration.
• Experience as a SAAS RN or equivalent role
• An ability to prepare written documentation using clear, concise and grammatically correct language.
• Intermediate IT skills.
• Previous experience as clinical RN in acute and in subacute care,
• Well-developed interdisciplinary assessment and reporting skills.
• Understands the issues and pressures to which the organisation has to respond.
• Identifies and supports where necessary the need for change

Selection Criteria Essential: Registered Nurse with current AHPRA registration.
