The Transition Care Program (TCP) is seeking an experienced, innovative and flexible Case Manager to work as a member of the TCP team.

The program is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long term care arrangements.

Key Responsibilities
The successful applicant will be flexible in terms of working across both residential and community areas. The Case Manager will need to demonstrate sound clinical assessment skills, and to have an understanding of the issues affecting patients with complex social and accommodation needs and an interest in aged care issues.

The successful applicant will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team.

Current AHPRA Registration as a Registered Nurse, Occupational Therapist or Physiotherapist, or qualified Social Worker, or suitable qualification as per Commonwealth Guidelines is essential.

Selection Criteria Desirable: Experience working in Aged Care
Essential: Must hold a current Victorian Drivers License
Desirable: Case management d/c planning experience
Essential: Registered Nurse, Occupational Therapist or Physiotherapist with current AHPRA registration, or qualified Social Worker, or suitable qualification as per Commonwealth Guidelines.
