POSITION SUMMARY

Ultrasound training program at Frankston ICU aims to train multidisciplinary team members in use of ultrasound in clinical practice. The program aimed at tiered experience if run in collaboration with Melbourne University. The role of Ultrasound Senior registrar (SR) is to assist in developing, implementing, and consolidating the ultrasound training program.

It is strongly encouraged that trainees undertaking Ultrasound SR year pursues higher ultrasound qualification such as Diploma in Diagnostic Ultrasound or similar.
Ultrasound SR will have dedicated 12 hours per week to carry out core activities related to Ultrasound training program.
In addition to ultrasound training and education time, Ultrasound SR will participate in ICU clinical roster performing clinical duties as ICU senior registrar.

College Activities

• Assist trainees and members of multidisciplinary team in achieving their Ultrasound training goals to improve clinical performance at Frankston ICU.

Hours of duty

• The Ultrasound SR will be rostered for 4 clinical shifts a fortnight (50 clinical hours /fortnight) with average shift length of 12.5 hours. Additional protected 26 hours/fortnight of ultrasound time dedicated for pursuing higher qualification and training goals will be incorporated in the roster.

• These ultrasound hours include attendance of cardiology Transoesophaeal Echocardiogram (TOE) lists, reporting time, planning meetings and Ultrasound teaching sessions to help further Ultrasound training and education program at Frankston ICU.

KEY RESPONSIBILITIES

Function as front line ICU clinician
Attend and lead ward rounds
Develop a curriculum and timetable
Initiate and assist care of patients within the ICU, as needed
Co-ordinate care with multiple teams of clinicians, as needed
Perform procedures within scope of practice, in accordance with Peninsula Health guidelines, as needed
Work closely with nursing staff to ensure patient flow
Lead morning handover amongst various teams involved in patient management

KEY RESULTS AREAS/MAIN PRIORITIES
OPERATIONAL/CLINICAL PERFORMANCE


• Ensure familiarity with the expected standards of performance in the role and actively contribute to own personal development.
• Maintain a good working knowledge of and adherence to standards and legislation relevant to the role (such as Child Safe Standards, Aged Care Act etc) and actively promote compliance to any such standards and legislation.
• Complete all mandatory training by the due date.
• Undertake any additional tasks as requested that reasonably fall within the scope of the position and classification.
• Ultrasound training and education for ICU junior medical staff (JMS) along with multidisciplinary team members.
• Assist in design and conduct of Ultrasound teaching program
• Develop a curriculum and timetable
• Supervise and support junior trainees in Ultrasound education
• Participate and support Ultrasound teaching program at Peninsula health
• Engage with Nursing Educators to deliver multidisciplinary Ultrasound teaching programme for nursing staff

SUPERVISION AND SUPPORT

• Graded clinical supervision by FCICMs with increment in clinical and non-clinical responsibility through the year (per CICM Guideline IC-4), based on performance and feedback. Consideration will be given to feedback from consultant group, junior medical staff, clinicians from other departments and nursing team in preceding period.
• Scheduled meetings of trainee with Supervisor of Training:
• Start of term meeting: discussion of expectations, goals, clinical as well as non-clinical duties. Any possible foreseeable barriers to achieving goals will be specified. Plans will be made to remove or mitigate such barriers.
Three month meeting: review of performance goals and level of supervision.
• Mid-term meeting: completion of formal In-Training Evaluation Report, review of performance and goals. Performance goals, clinical duties, non-clinical portfolio

• Maintain up-to-date immunisation status related to own health care worker category.
• Ensure that the principles of general and patient manual handling are adhered to.
• Morbidity and Mortality reviews
• Rapid response call reviews
• In depth case reviews and Root cause analyses
• Design and ethical conduct at least one project that can be classified as a QA activity. Support for such an activity will be readily provided. However, ethical design and conduct is responsibility of senior registrar/fellow.
• Take lead in departmental multidisciplinary morbidity and mortality meetings
• Participate in quality improvement projects
• Participate in auditing practice as part of QA activity
Develop clinical practice guidelines.

SELECTION CRITERIA & DESIRABLE CRITERIA

Clinical competencies

• Ultrasound SR must have completed FCICM primary examination. Trainees who are at an advanced stage of ICU training programme will be preferred.
• Basic Ultrasound Knowledge and skills are preferred but not a pre-requisite.
• Trainees with critical care careers with demonstrable special interest in Ultrasound are preferred.

Non-Clinical Competencies:

Adhere to the expectations of professional behaviour as per the Code of Conduct of CICM (IC-21) and Medical Board of Australia
• Develop capacity for self-regulation of behaviours, emotions, and attention to maintain performance and self-care
• Deliver high quality care with integrity, honesty and compassion through the application of sound judgement and a knowledge of ethical principles and the law
