Position Summary The Cardiac Nurse works within the Integrated care and Dental stream within Peninsula Health Community Health. This stream targets the health needs of adults 65 years and older, younger adults with a disability, and adults with a chronic condition and/or psycho social issues. This includes the indigenous community and CALD groups.
The role of the Cardiac Nurse within the Early Intervention in Chronic Disease program includes treatment services, early intervention and health promotion and prevention programs including self-management strategies to meet the needs of the target group within a social model of health by; • Promoting wellness • Encouraging people to share in responsibility for their wellbeing • Encouraging community participation • Liaison and coordination with service providers
