Position Summary

We currently have an exciting opportunity for an experienced and person centred focused speech pathologist to join our allied health Outreach Service for 5 weeks to cover long service leave. Days and hours are negotiable up to a maximum of 64 hours per fortnight.

The Outreach Service provides high quality evidence based speech pathology service to eligible consumers in their home and community environments and at community health centres. Innovative person centred care may include supports for people (and their carers) to maintain a maximum level of independence and safety in their community, through the provision of swallowing and communication assessment, management of complex communication and swallowing disorders, education for clients, carers and staff, low intensity therapy and prescription of high and low technology and non-electric communication aids.

The Outreach Service is a HACC and Commonwealth funded program and registered NDIS service provider, providing allied health services across the beautiful Frankston and Mornington Peninsula Local Government Areas. The team is integrated across Community Health Centres on the Peninsula. The Outreach Service provides a supportive and dynamic team environment with an emphasis on innovation. The Outreach Allied Health team comprises Physiotherapy, Speech Pathology, Occupational Therapy, Allied Health Assistant and Carpentry Services.

We are looking for:
• An enthusiastic, experienced and motivated Speech Pathologist with current SPA registration
• Current Victorian Drivers Licence
• Ability to provide person centred care in the home environment.
• Experience in assessing and managing clients with complex needs.
• Exceptional time management skills and ability to work unsupervised in the community
A working knowledge of the My Aged Care System and NDIS will be advantageous.

Key Responsibilities

• Provide quality, safe, evidence based speech pathology services within a multi-disciplinary team.
• Develop and maintain effective partnerships with patients, families, carers and multidisciplinary team members in support of optimal patient safety and person-centred care delivery.

Selection Criteria Essential: Current practising membership of Speech Pathology Australia
Essential: Demonstrated excellence in adult community based Speech Pathology service provision in accordance with person-centred care principles
Essential: An understanding of Peninsula Health Community Health principles and values
Essential: Excellent communication skills (verbal and written) with clients, families and multidisciplinary teams
Essential: Commitment to multidisciplinary team collaboration
Essential: Demonstrated effective time management and organisational skills and ability to work unsupervised in the community
Essential: Current Victorian driver’s license and willingness to transport clients as necessary
Essential: Commitment to Continuing Professional Development and associated evidence based practice
Desirable: Experience and expertise in the provision of Alternative and Augmentative Communication (AAC) options.
Desirable: Experience in working with people with progressive neurological disorders
