What You Will Be Doing

Peninsula Health has a vacancy for a dynamic and experienced Clinician to lead the PH Home Care Package program empowering vulnerable clients with complex health and social needs to remain independent in the community. The Team Leader will be based at Mornington Community Health and work with participants and Community Health staff across the beautiful Local Government areas of Frankston and the Mornington Peninsula.

The Team Leader will have exceptional business and financial management skills, will maintain a small ongoing clinical caseload and provide supervision and mentoring to all program staff. The Peninsula Health HCP program is currently approved for 120 Care Packages and maintains a niche specialty for complex marginalized clients. We pride ourselves on the provision of highly qualified and experienced Care Managers and wrap around health support. The HCP team currently comprises four Care Mangers, a Financial Support Worker and a Service Coordinator.

Community Health provides a dynamic, supportive and social workplace with a range of multidisciplinary clinicians. Training, professional development and opportunities to lead service development and quality improvement activities are integral to the Outreach Program. What You Need

• Tertiary qualifications in Allied Health or Nursing with current APHRA registration (or registration with relevant professional body)
• Demonstrated capability in business and financial management
• Demonstrated advanced knowledge and skills in the provision of evidence based services in a community setting.
• A demonstrated commitment to the Commonwealth Client Directed Care Principles
• Demonstrated clinical leadership and supervision of staff to promote professional competence and foster individual staff development.
• A current Victorian drivers licence
• A thorough understanding of the Commonwealth Aged Care Legislation
• Working knowledge of the Home Care Package Program and practice guidelines
• Excellent skills using data and reporting on service outputs and outcomes.
• A flexible independent approach to their work with high level negotiation and problem solving skills
