Position Summary

The Forensic Mental Health in Community Health Program (FMHiCH) program is designed to support the delivery of collaborative, safe, high quality primary forensic mental health services and facilitate referral and engagement with other relevant health services to support improved mental and physical health outcomes for our clients. Individuals eligible for referral from CCS into the FMHiCH program are adults (over 18 years) who meet the following four criteria:
• Are people on Parole or on a Community Correction Order
• Who have a primary diagnosis of a moderate severity mental illness or disorder
• Are assessed as being at moderate to high risk of re-offending
• Who have a court mandated condition on their Parole or Community Correction Order to undertake Mental Health Treatment and Rehabilitation.

The overall aim of the Program is to proactively engage eligible individuals and deliver a forensic mental health service to address their mental health needs. As part of offering a holistic service other health issues should be identified and appropriate referrals made to services offered by the community health service and to other relevant providers. Health issues that could be identified include chronic illnesses such as respiratory disease, diabetes, heart disease, dental issues, and social support needs such as housing, employment, family issues. Collaborative Practice with the Community Correction Services case manager about care planning and referrals for social support will be essential as part of the framework for successful service delivery.


Key Responsibilities

• Work collaboratively with Corrections in both Frankston & Rosebud to deliver flexible intake services responses to clients of the Forensic Mental Health Program. This will at times include colocation with Corrections and outreach based client work to meet the needs of this cohort.
• Deliver evidence-based mental, social and physical assessments of clients in the workplace or in the community that will lead to recommendation of a course of treatment and services to individuals in the defined cohort that are timely, culturally safe and responsive.
• Assessment will include consideration of trauma and associated therapeutic and social support, cultural, social and emotional wellbeing needs of the client.
• Assess level of urgency in situations of psychiatric crisis assessment and undertake a course of action to resolve or ameliorate the crisis accordingly.
• Assess eligible clients’ capacity for self-management of their mental illness and any other health issues
• Prepare care plans in collaboration with clients and their nominated support person(s).
• Provide counselling and therapy to clients.
• Establish/maintain strong partnerships with local Aboriginal organisations to ensure the delivery of culturally competent and safe care to Aboriginal people
• Adherence to Balit Murrup social and emotional wellbeing model (where Aboriginal clients are involved)
• Facilitate strong links and referral pathways with our acute based mental health services to streamline access to specialist services in a step up, step down approach to mental health care
• Facilitate access to social support services (such as housing, employment, education/training and family services) through service partnerships and supported or assertive referral (liaising with the CCS case manager).


Essential Criteria

Qualified and Registered, with relevant National Board.

Nursing: Registered Nurse with appropriate post basic qualification in mental health nursing who are registered with AHPRA

Selection Criteria Essential: Nursing: Registered Nurse with appropriate post basic qualification in mental health nursing who are registered with AHPRA
Essential: Extensive experience in the mental health field and/or community mental health care
Essential: Demonstrated working knowledge and understanding of the Victorian Mental Health Act 2014, the National Standards for Mental Health Services 2010, the National Standards for the Mental Health Workforce 2002 and the Australian Council on Healthcare Standards including EQuIP.
Essential: Proven ability in the management of clients with a major mental illness and in the techniques of assessment, crisis intervention, counselling and psychosocial rehabilitation
