The Community Health Outreach Service has an opportunity for a dynamic forward thinking Registered Nurse to lead the Mobile Integrated (M.I.) Health Team.

The M.I. health team utilizes an assertive outreach model to assist people with complex health needs in the LGA areas of Frankston and the Mornington Peninsula Shire who are primarily vulnerable, homeless, or living in insecure housing. The aim of the service is to link people into housing, health and welfare services via a case management model.

The program comprises a range of funded services including Community Nursing, the Community Connections Program; The Supporting Vulnerable Victorian Initiative (SAVVI) & Pension Level Project (PLP); Aboriginal Access and Support; and the management of the Commonwealth funded Home Care Package (HCP) service.

The MI Health team leader also provides leadership to a number of projects aiming to improve the health and wellbeing of people who are homeless, or living in insecure housing with complex health needs such as the Parwan Crs Transitional Housing Property and the WAYSS Rooming House Project.

Key Responsibilities:
Operationally responsible for the M.I. Health Team to ensure the delivery of high quality services on a daily basis

Management of a clinical caseload and provision of nursing expertise within the various M.I. Health programs

As a member of the leadership group facilitates on-going quality improvement in service development and delivery and adjustment to relevant Victorian and Commonwealth reform legislation.

Essential Criteria:
- Qualifications as an Registered Division 1 Nurse and current registration with APHRA
- Experience in the planning and delivery of outreach health and/or welfare services, particularly to people with complex chronic health conditions, who are homeless or at risk of homelessness.
- Knowledge of team building processes and the ability to promote team development. Experience in the management of community based services and staff
- Current Victorian Driver’s License
- Highly developed communication skills including negotiation, conflict resolution and creative problem solving.
- Demonstrated commitment to the principles of community health including the Social Model of health and Health Promotion.

Selection Criteria Essential: Qualifications as a Registered Div 1 Nurse

Desirable: An understanding of the pension level SRS sector and knowledge of Home Care Packages and Aged Care Reform
Desirable: Ability to understand and work within a change management process.
