Position Summary An exciting opportunity now exists for a Registered Mental Health Nurse with relevant mental health qualifications, and or experience to join our Aged Persons Community Mental Health Team.
The Aged Persons Community Mental Health Team (APCMHT), incorporating the Intensive Community Assessment and Treatment Team (ICATT), is a multidisciplinary team which provides a community-based specialist mental health service to clients living in the Peninsula Health Mental Health Service (PHMHS) catchment who are over the age of 65 experiencing severe and persistent mental health disorders.
Key Responsibilities This specialist multi-disciplinary and integrated service provides care co-ordination, case management and community treatment, usually in the person’s place of residence and clinicians are available to provide family support, counselling and carer education. Care co-ordination for consumers is via assertive outreach or at satellite clinics based at Hastings, Mornington and Rosebud Hospital.
The mental health clinician works closely with other care agencies, general medical practitioners and residential aged care facilities.

Essential Criteria The successful applicant will be a registered nurse clinician with advanced interpersonal skills and preferably have a broad range of experience in mental health and in particular aged persons mental health. They should possess skills in case management, risk assessment and recovery; and have a thorough understanding of the mental health system, as well as other community resources available.
