Peninsula Health is seeking an experienced and innovative Grade 2 Social Worker.

This is a dynamic, challenging and rewarding position, suited to a Social Worker with a commitment to patient centred care. The position provides exciting opportunities for a Social Worker looking to develop his/her career in the health system in particular at Rosebud Hospital . Peninsula Health has an Integrated Social Work service with opportunities for staff to gain experience across a range of service areas in Acute and Sub-Acute care.

The successful applicant will provide, as part of an inter-disciplinary team, actions and planning, to link patients and their families/carers, with appropriate services that support safe discharge from hospital.

Applicants must be eligible for membership of the AASW, have excellent communication and interpersonal skills, and an interest in working in a healthcare setting.

Essential: Excellent skills in comprehensive assessment, discharge planning and counseling
Essential: Commitment to continued professional development.
Essential: Previous hospital experience.
Essential: Demonstrated ability to work collaboratively.
Essential: Advanced demonstrated knowledge and application of Social Work theory.
Essential: Demonstrated ability to supervise staff and/ or students.
Essential: Ability to manage a demanding workload, and effectively prioritise tasks.
Essential: Computer literacy.
