What You Will Be Doing
Peninsula Health currently has an opportunity for an experienced and person centred focused Clinical Nurse Consultant to join our Acute Cognition Service on a part time basis. Collaborating closely with a multidisciplinary team, the CNC will provide a coordinated and proactive approach to cognition management. This will occur through consultation, assessment & support for patients with cognitive impairment and education/support for staff.

What You Need
* Provide clients and staff expert advice, guidance and clinical support in cognition management and behaviours of concern. * Promote assessment of patients with delirium and provide consultancy and advice in collaboration with medical staff. * Provide timely assessment and development of recommendations/care plans for patients with complex behaviours of concern. * Actively participate in appropriate working parties and steering committees to improve the care of people with a cognition impairment within the hospital setting.
We anticipate you will have:
• Registered Nurse with current registration certificate • Recent acute experience in providing care for people with cognitive impairment • Commitment to the ongoing development of cognition as a specialty area • Sound knowledge and skills in assessment and management of people presenting with behaviours of concern • Problem solving skills
