What You Will Be Doing

The Homelessness Outreach case manager position is an outreach role which assists people facing barriers to accessing services, and who have diverse characteristics, including Aboriginal and Torres Strait Islander people, to engage with services, maintain their independence and continue to live safely at home. The client population seen by this role are people who are residing in insecure or unsuitable housing, experiencing housing stress or experiencing homelessness. The position sits within the Homelessness and Health Outreach Service.


What You Need

We anticipate you will have:
• Bachelor’s degree in Social Work or Nursing, or qualifications for Community Development Worker.
• Experience in working with clients with complex needs. This may include community-based services, such as the housing sector, AOD or Mental Health.
• Experience working in health care, community health or outreach settings.
• An applied understanding of the issues and implications of homelessness for clients with complex health and social needs.
• Experience in case management or similar modalities.
• Highly developed communication and documentation skills.
• Valid driver’s license.

*Please refer to the relevant salary circular PROPOSED NURSES AND MIDWIVES (VICTORIAN PUBLIC HEALTH SECTOR) (SINGLE INTEREST EMPLOYERS) ENTERPRISE AGREEMENT 2020 - 2024 or ALLIED HEALTH PROFESSIONALS (VICTORIAN PUBLIC SECTOR) (SINGLE INTEREST EMPLOYERS) ENTERPRISE AGREEMENT 2021 – 2026 for above mentioned classifications for salary range. Sub-grading will be based on years of experience.

This role must be fully vaccinated for Covid-19 and meet the requirements of the Peninsula Health immunisation policy.

Selection Criteria Essential: Bachelor’s degree in Social Work or Nursing, or qualifications for Community Development Worker.
Essential: Experience working in health care, community health or outreach settings.
Essential: Valid driver’s license and ability to transport clients where required (fleet cars available).
Essential: Highly developed communication and documentation skills and excellent IT, computer systems and Microsoft Office skills and the ability to competently learn and process new information and systems.
Essential: Well-developed organization, time management, planning and priority setting skills.
Essential: Experience working in health care, community health or outreach settings.
