Advertisement text Peninsula Health has an opportunity for an experienced Nurse/Team Leader to join the MI Health Team within Community Health.

This is an exciting opportunity to manage a dynamic and highly skilled team providing a range of services to clients who have complex health needs who are homeless or at risk of homelessness. The Program comprises Nursing, the Community Connections Program, The Supporting Vulnerable Victorian Initiative (SAVVI) and Pension Level Project (PLP), Aboriginal Access and Support, Rooming House Project, and the Home Care Packages Service.
This is a 6 month Long Service Leave replacement. Hours negotiable with a minimum of 64 hours per fourtnight.

Key Responsibilities

•Operationally responsible for the MI Health Team to ensure the delivery of high quality services on a daily basis

•Management of a clinical caseload and provision of nursing expertise within the various MI Health programs

•As a member of the leadership group facilitates on-going quality improvement in service development and delivery.

Essential Criteria

•Qualifications as a registered Div 1 Nurse

•Current Victorian Driver’s License

•Experience in the planning and delivery of outreach health and/or welfare services, particularly to people with complex chronic health conditions, who are homeless or at risk of homelessness.

•Experience in the management of community based services and staff

•Highly developed communication skills including negotiation, conflict resolution and creative problem solving.

•An understanding of, and commitment to, the principles of community health including health promotion and the Social Model of health.

Desirable Criteria
An understanding of the pension level SRS sector and knowledge of Home Care Packages and Aged Care Reform.

•Knowledge of the local demographics of the region and the impact upon the planning and delivery of services.

Selection Criteria Essential: Div 1 Registered Nursing qualifications and current registration with APHRA
Essential: Current Victorian drivers licence
