The Short Term Support Coordinator sits within Peninsula Health’s Community Care Program. This program and is based on the Wagner Chronic Care Model, an evidenced based systems framework. The aim of this position is to shorten the length of time patients need to stay in hospital and to prevent unplanned readmissions. This is done by arranging appropriate brokered and community supports and services for those who need extra assistance to return home.

The role includes the provision of information and assistance to referrers and consumer/ community enquiries, and is based on a platform of superior communication skills and knowledge of relevant health and community services. It involves facilitating collaborative integrated care planning across acute and community based services.

The role will be remunerated under the Health Professionals, Health and Allied Services, Managers and Administrative Officers EBA however applications are welcomed from health professionals with tertiary qualifications in health.

Key Responsibilities

• Work with the referring physician, patient/family, acute and sub acute hospital health care professionals, case management associates, external agencies when indicated and other health care providers to promote a smooth, efficient and customer focused transition from in-patient care to community care services.
• Enhance the client experience through effective and efficient patient care coordination by activating and organising services as identified in the care plan and post discharge phone calls.
• Develop a strong knowledge of services available both within Peninsula Health and within the community sector and provide relevant information to consumers.
• Provide information and referral to services in accordance with funding guidelines and established service guides.


Essential Criteria

• Tertiary qualifications related to health service delivery
• Experience and knowledge regarding chronic disease
management
• Experience working with at-risk and vulnerable people with complex
needs.
• Well-developed written, verbal, interpersonal and computer literacy and
communication skills
• Understanding, knowledge and experience implementing service
coordination frameworks within a health setting
• Experience in working both independently and as a part of a multi-
disciplinary team, with the ability to work effectively, supportively and
flexibly.
• Current Victorian Driver’s Licence.
