Position Summary The Transition Care Program (TCP) is seeking an experienced, innovative and flexible Registered Nurse (RN) to work as a Clinical Nurse Consultant (CNC) as a part of the TCP team on a permanent full time basis.
As a CNC you will be responsible for the provision of service prior to admission, and within the TCP episode of care including advanced clinical assessments, effective discharge planning and provision of information/support and resources to the TCP client and relevant internal and external stakeholders.
In addition you will provide leadership and support to colleagues within TCP, and assist with ongoing education and liaison about the program.
About TCP The TCP is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long term care arrangements.
TCP has a dynamic team, with a focus on the provision of quality care. The Team enjoys excellent working conditions, well equipped facilities, and opportunities to participate in quality improvement and research.
Key Responsibilities As the CNC you will be responsible for providing an excellent, evidence-based clinical service, for patients with a broad range of comorbidities. Experience and competence across inpatient, residential care and community settings is crucial and will be highly regarded.
Essential Criteria You will need to be flexible in terms of working across inpatient, residential and community areas. You will need to demonstrate sound clinical assessment skills, and have an understanding of the issues affecting patients with complex functional and cognitive needs and an interest in aged care issues.
You will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team.
