Position Summary
The Community Health Independence Nurse Manager will be part of the Community Health Senior Management Team. This position incorporates staff from the Health Independence Program to prevent the need for a hospital presentation or stay, with many people accessing them directly from the community. The Nurse Manager will be responsible for managing the Community Health Independence screening, recruitment, short term support and clinical nursing staff. These nurses provide a range of clinical services that deliver healthcare to support the transition from hospital back to community. Staff provides outreach services in community settings, residential aged care facilities and in client’s homes.

Key Responsibilities

• Provide operational management and leadership within the Community Health Independence programs.

• Contribute to the strategic development of community health as part of the senior community health management team.

• Maintain and continue to develop robust Clinical Governance of service provision.

• Ensure Peninsula Health’s Community Health Independence services provide specialised client-centred co-ordination in the community/acute and sub-acute setting through an integrated response of hospital and community services.

• Ensure that funded programs specific to Peninsula Health’s Community Health Independence Program meet targets as per funding agreements.

Essential Criteria
•General Registration as a Registered Nurse with Australian Health Practitioner Agency
• Current Victorian License
• Extensive clinical, operational and management experience
• Experience in Organisational Cutural Change and Development
