Position Summary

The Mt. Eliza Assessment Service is seeking to employ an enthusiastic and motivated Comprehensive Assessor to complement the existing multi-disciplinary team on a fixed term, part time basis.

Practicing within the framework of the Aged Care Assessment Program (ACAP) and within Peninsula Health organisational requirements, the Comprehensive Assessor will provide a comprehensive and independent assessment of the aged client (and some younger adults when appropriate), and with the client, develop care plans that promote the restoration of function and/or maximise independence.

The Comprehensive Assessor will also review support plans and facilitate referrals or provide service coordination for older people to engage the range of community or hospital based services in order to meet their specific needs.

The successful candidate will demonstrate an ability to work autonomously as an effective member of the team, with commitment to the delivery of coordinated and innovative practice within the community aged care service. You will be dedicated to best practice principles and be active towards achieving personal development and team progression.

You will be required to work across the range of health and community sectors to provide sustainable, well-managed and efficient service delivery, together with contributing to the development of strategies that improve Aged Care Assessment Service. This includes an ability to adapt and operate within a changing service environment.

Key Responsibilities

• Be aware of and work in accordance with organisational policies and procedures including award conditions, occupational health and safety and equal employment opportunity.
• Operationally support the MEACAS Program Manager and work with the Senior Team to lead and develop systems and processes to ensure that MEACAS meets its performance targets and requirements.
• Ensure services are optimally delivered in the most appropriate setting in a timely manner, caring for clients and meeting key performance targets.
• Contribute to and lead in the development of a positive workplace that fosters excellence in customer service.
• Build and expand personal skill levels to ensure that clients receive the best possible service reflecting up-to-date, evidence based professional practice.
• Conduct comprehensive assessments of the target client group and work with the client towards the formulation of a support plan
• Provide clear, concise communication (verbal and written) of assessment outcomes, document support plans and recommendations to clients, their carers and any involved or relevant services
•Function effectively and efficiently within the current operating system for the ACAS which includes working online and offline as appropriate
•Operate to fulfil the range of ACAS practice requirements as indicated by current Commonwealth guidelines
•Manages time appropriately across all clinical and administrative tasks
•Demonstrates commitment to achieve and maintain the range of Commonwealth directed targets (Key Performance Indicators) and targets relevant to the ACAS.

Selection Criteria Essential: Registered General Nurse, Occupational Therapist or Physiotherapist with current AHPRA Registration, Social Worker qualifications or suitable qualifications as per Commonwealth Guidelines
Essential: Current Victorian Driver's License
Essential: Understanding of the ACAS role and guidelines
Essential: Computer literacy and proficiency
Essential: Demonstrated ability to work autonomously as an effective member of a multidisciplinary team
Essential: Demonstrated knowledge of the aged care client and issues related to ageing
Desirable: Experience in current Aged Care Assessment Practice, My Aged Care or demonstrated knowledge of the current Aged Care Assessment Service provisions
Desirable: Experience in working in the community setting or sub-acute bed based services
Desirable: Ability to demonstrate ongoing commitment to service development
