Position Summary
The Transition Care Program (TCP) at Peninsula Health sits within the Community and Continuing Care cluster and provides a comprehensive range of multidisciplinary services across multiple service sites throughout the Mornington Peninsula.
The program provides an extensive range of services, such as nursing care/personal care and case management as well as low intensity therapy provided by Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics and Social Work.
TCP aims to minimise inappropriate extended hospital lengths of stay and prevent unnecessary presentations or admissions to the Emergency Department.
The Transition Care caseload comprises both Residential and Home-based service delivery, and includes musculoskeletal, neurological and other conditions. Patients may be managed individually or via specialised programs such as Age Strong, Walk Tall, Balance Group and by referral to CRP. The Case Manager will be capable of managing a range of clinical conditions and social scenarios.

Key Responsibilities
• Develop an awareness and understanding of all relevant codes of practice and policies, including all Peninsula Health policies and procedures.
• Provide a high standard of clinical/social assessment and management across a broad range of aged care clinical streams including orthopaedic, neurology, respiratory care, dementia and falls.
• Provide clinical leadership to continually improve inter-disciplinary processes of care, patient outcomes and service objectives.
• Ensure all documentation is completed in accordance with the department standards and Peninsula Health policies and procedures.

Essential Criteria
• Registered General Nurse, Occupational Therapist or Physiotherapist with current AHPRA Registration Social Worker qualification or suitable qualification as per Commonwealth Guidelines
• Previous experience as a Clinician
• Demonstrated competence in a specific Allied Health/RN area of practice
• Capacity to supervise student
• Knowledge of / experience in Aged/Residential Care settings.
• Current Driver’s Licence
• Demonstrated participation in Professional Development.
• Interest and/or experience in Quality improvement and research activities
• Computer literate
• Proven ability to fulfil expectations in key result areas
• Leadership capacity
The Transition Care Program is seeking an experienced, innovative and flexible Case Manager to work as a member of the TCP team.

The program is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long term care arrangements.
