Position Summary

The Outreach Service is a HACC and Commonwealth funded program providing service across the beautiful Frankston and Mornington Peninsula Local Government Areas. The team is integrated across Community Health Centres on the Peninsula.

Peninsula Health has an exciting opportunity for an Allied Health Care Co-ordinator to provide short term transdisciplinary Case Management to clients and their family/carers of all ages, with disabilities and complex care needs to navigate the health care system. Intervention is focused on promoting independence, capacity building and self-management while aiming to improve health, social and emotional well-being. This may include support to access a Commonwealth Home Care or NDIS Package. The position is permanent, 32 hours per fortnight. Numeration will be based on the successful applicant's formal qualifications.

Key Responsibilities

• Undertake client assessment and screening in a range of settings, including home visits across the Frankston and Mornington Peninsula Shire catchments.
• Assessment of health, personal, social and domestic care requirements to support independence and self-management
• Provision of reports detailing clients service and support requirements
• Advocacy, referral and support in accessing care through a variety of community based and other agencies

Essential Criteria

• Allied Health or Equivalent Qualification
• Current Victorian Drivers Licence
• Understanding of Aged Care Reforms and the National Disability Insurance Scheme
• Good negotiation, communication and listening skills
• Broad understanding of neurological, mental health and congenital disabilities, and general care requirements across the lifespan
• Demonstrated ability to work autonomously, with exceptional time management and organisational skills
