Position Summary

Are you an enthusiastic physiotherapist who would like to work with a diverse caseload in a collaborative, multidisciplinary community health team on the beautiful Mornington Peninsula?

Peninsula Health Community Health has an employment opportunity for a dynamic Grade 2 Physiotherapist on a permanent part time basis – 4 days per week - in the Early Intervention in Chronic Disease (EIiCD) Program. The role will be predominately based at Mornington Community Health site.

The Early Intervention in Chronic Disease (EIiCD) Program predominately targets the health needs of clients with a chronic condition; and / or disability or psycho social needs. The EIiCD program includes treatment services; early intervention and prevention programs including self-management strategies to meet client needs within a social model of health framework.

We require an experienced, innovative physiotherapist to deliver high quality evidence based clinical care for a broad range of clients in a community health setting. The role involves working within an integrated, multidisciplinary team to work both with individual clients and within group program settings including aquatic therapy.

The role requires skills and experience in performing client centred assessments and care plans, and the ability to provide a flexible treatment approach focused upon self-management, and outcome evaluation. Peninsula Health is affiliated with Monash University and there are also opportunities for the supervision of Physiotherapy students in this role.

Key Responsibilities

• Provide quality, safe, evidence based physiotherapy services within a multi-disciplinary team.
• Utilise a client centred approach to assessment and care planning ensuring that the broader health needs of the client are included.
• Build and expand professional skill levels to ensure that clients receive the best possible service reflecting up to date evidence based practice.
• Work within an agreed scope of practice in accordance with Peninsula Health policy governing credentialing, scope of practice and introduction of new interventions.
• Demonstrate positive workplace behaviour and relationships at all times.

Selection Criteria Essential: Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
Essential: Eligible for membership or Member of the Australian Physiotherapy Association
