Position Summary
This role sits within Peninsula Health’s (PH) AOD Program and is based in the Community Health sites, including Frankston, Rosebud and Hastings. The position involves supporting complex AOD clients throughout their treatment journey using a care coordination approach. It involves providing individualised and flexible support that supplements clients AOD treatment, and liaising with other key staff and services that are involved in clients’ care.

Key Responsibilities
• Coordinate treatment planning and care in accordance with clients’ recovery goals
• Support clients to access other health/human/support services
• Create and sustain strong inter-agency connections and integrated service responsive to the holistic needs of clients
• Work with clients, their families and other key stakeholders to develop Individual Recovery Plans.

Essential Criteria
• Qualification appropriate to the position such as Certificate IV Alcohol and Other Drugs work
• Demonstrated experience working with at-risk and vulnerable people with dual diagnosis
• Demonstrated experience in working both independently and as a part of a multi- disciplinary team, with the ability to work effectively, flexibly and supportively
• Demonstrated knowledge in providing care coordination or case management to complex and high risk clients
• Current Victorian Driver’s Licence
