Position Summary

Peninsula Health Mental Health Service are seeking an energetic Community Mental Health Intake Worker to work within the Mornington Community Mental Health Team.

The successful applicant will be responsible for ensuring the smooth transition of client referrals to community mental health services from telephone triage, inpatient units, access and assessment team, consultation liaison emergency department and general hospital.

Key Responsibilities

• Positions at this level utilise advanced clinical knowledge and skills to provide quality mental health services within their professional scope of practice.
• Plan, implement, and document comprehensive ongoing care planning for clients in accordance with evidence based best practice standards and Peninsula Health Values.
• Coordinate the transfer of care of clients for youth, adult and aged community mental health service.
• Build good inter service relationships in order to achieve best client outcomes.
• Maintain up to date knowledge of PHMHS.
• Ensure the safety of the public, patients and staff in relation to the provision of a safe environment from both OH&S and Infection Control Perspectives.
• Complete all mandatory training as per policy and participate in organised training and development activities.
• Be responsible for maintaining contemporary knowledge of the mental health specialities.
• Liaise with internal and external service providers as required

Selection Criteria Essential: Degree in social work or other relevant qualification, with relevant qualification and approved for membership of the Australian Association of Social Workers
Essential: Qualified and Registered, with relevant National Board, Clinical Psychology for Psychologist; Mental Health nursing qualification such as a post graduate qualification in psychiatric/mental health nursing or a specialist undergraduate psychiatric/mental health nursing program).
Essential: Sound knowledge of the CMI database
Essential: Demonstrated working knowledge and understanding of the Victorian Mental Health Act 2014, the National Standards for Mental Health Services 2010, the National Standards for the Mental Health Workforce 2002 and the Australian Council on Healthcare Standards including EQuIP
Essential: Proven ability in the management of clients with a major mental illness and in the techniques of assessment, crisis intervention, counselling and psychosocial rehabilitation
Essential: Extensive experience in the mental health field including acute inpatient services and/or community mental health care
Essential: Demonstrated ability to prioritise clients care needs based on presentation as well as workload including escalating client care needs as required.
Desirable: A current Victorian Drivers Licence
Desirable: Previous experience in mental health triage/intake
Desirable: Is able to identify continuous improvement opportunities for improved patient/consumer outcomes.
