Position Summary
The Acute Adult Inpatient Unit (2 West) is a 29 bed unit based at the Frankston Hospital site of Peninsula Health. The Grade 1 Social Worker works under the direction of the Senior Social Worker and Nurse Unit Manager. In conjunction with the multidisciplinary team, the Grade 1 Social Worker provides recovery orientated care to inpatient consumers of the service and their families/carers. In order to achieve their position outcomes the Grade 1 Social Worker will develop effective partnerships with all staff including medical, nursing and allied health staff.

The Grade 1 Social Worker is a valued and integral member of the Peninsula Health multidisciplinary team, demonstrating flexibility in caring for consumers and their carers across the organisation. The Grade 1 Social Worker works competently within their scope of practice to deliver safe, evidence based person centred care to achieve optimal health outcomes.

Key Responsibilities
• Provide quality focussed care, to consumers and their families/carers through competent clinical treatment, assessment and discharge planning skills.
• Able to resource and provide information to assist a consumers’ recovery.
• Provide effective communication, both verbal and nonverbal, and documentation skills.
• Able to work effectively in a multidisciplinary team.
• Demonstrate assessment/counselling skills.
• Demonstrate experience and interest in continuous quality improvement.
• Facilitate recovery orientated groups on the inpatient unit
• Contribute to educating and raising awareness of the Social Work role within Peninsula Health.
• Demonstrate flexibility/initiative and a keenness to learn

Essential Criteria
• Member or eligibility of the AASW
• Demonstrated commitment to delivering high quality outcomes for consumers and families
• Skills in comprehensive assessment, discharge planning and counselling
• Commitment to continued professional development
• Excellent interpersonal and communication skills
• Demonstrated ability to work collaboratively within a team
• Prepares written documentation using clear, concise and grammatically correct language
• Skills to facilitate recovery orientated groups
