This is an excellent opportunity for a suitably qualified Physiotherapist to join a multidisciplinary team at Peninsula Health Falls Prevention Service that offers a supportive environment dedicated to best practice in falls prevention and healthy ageing.

Position Summary
The Physiotherapist is a valued member of the multi disciplinary falls prevention team at Peninsula Health and delivers specialist home based falls risk assessments and specialist physiotherapy assessment at the centre based falls clinics on a regular basis and short term home based or centre based physiotherapy interventions as required.

Key Responsibilities
• Work within the scope of practice of a specialist falls prevention clinician
• Triage referrals
• Deliver timely specialist falls risk assessments
• Develop an individualised falls management plan in collaboration with the client
• Provide short term physiotherapy interventions where required,
• Conduct a review of client’s progress in approximately 3 months from initial assessment
• Timely discharge with appropriate hand over where indicated
• Provide safe and effective care to patients within the designated caseload
• Ensure a high standard of service is delivered at all times
• Demonstrate positive workplace behaviours and relationships at all times
• Maintain up to date professional development
• Coordinate and participate in relevant quality improvement activities.

Essential Criteria
• Current registration with the Physiotherapy Board of Australia (via AHPRA).
• Current Victorian Driver’s licence.
• Experience in delivering home based assessment/intervention
• Advanced ability to assess and manage falls risk in community dwelling elderly people considering all relevant aspects of the person and their environment
• Demonstrated competence in physiotherapy assessment and rehabilitation of elderly clients at risk of falls.
• Demonstrated commitment to professional development, quality improvement and evidence based practice.
• Computer literacy.
• High level of verbal and written communication skills.
