Position Summary

We are seeking a highly motivated and experienced Physiotherapist or Occupational Therapist for a fixed term part time position to join our Outreach Service. Hours and days are negotiable.

Ideally you will have experience in assessing and managing clients with complex needs, demonstrated excellence in physiotherapy or occupational therapy theory and practice, prescriber status with SWEP and a commitment to person centred care. A working knowledge of the My Aged Care System and NDIS will be advantageous.

The Physiotherapist assists eligible clients by providing home based exercise programs, short term management of recent injuries, reconditioning programs, mobility and gait training, assessment for gait aids, waitlist management projects and groups to best meet the needs of the population.

The Occupational Therapist assists eligible clients by providing comprehensive home assessment; specialist recommendations regarding assistive equipment and home modifications; client and carer support and education and referral

The Outreach Service is a HACC and Commonwealth funded program and registered NDIS service provider, providing allied health services across the beautiful Frankston and Mornington Peninsula Local Government Areas. The team is integrated across Community Health Centres on the Peninsula. The Outreach Service provides a supportive and dynamic team environment with an emphasis on innovation.

Ideally you will have experience working in Community Health and a sound working knowledge of the Social Model of Health.

We are looking for:
• An enthusiastic and motivated Physiotherapist or Occupational Therapist with current APHRA registration
• Current Victorian Drivers Licence
• Ability to provide person centred care in the home environment.
• Experience in assessing and managing clients with complex needs.
• Exceptional time management skills and ability to work unsupervised in the community
• Excellent verbal and written communication and report writing skills.
• Experience with home visiting, equipment prescription and environmental modifications

Key Responsibilities

• Provide quality, safe, evidence based physiotherapy services within a multi-disciplinary team.
• Utilise a client centred approach to assessment and care planning ensuring that the broader health needs of the client are included.
• Participate in collaborative program planning and implementation.
• Work within an agreed scope of practice in accordance with Peninsula Health policy governing credentialing, scope of practice and introduction of new interventions.
• Demonstrate positive workplace behaviour and relationships at all times.

Selection Criteria Essential: Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
Essential: Eligible for membership of the Australian Physiotherapy Association or the Occupational Therapy Association of Australia depending on discipline.
Desirable: Current registration as a prescriber with the State Wide Equipment Program (SWEP), preferably with some Amber and Red credentials.
Essential: Sound knowledge of current Physiotherapy or Occupational Therapy theory and practice AND an ability to demonstrate use of this in the home and community environments
Essential: Understanding of client self- management principles and the Commonwealth Reablement Model and Victorian Active Service Model.
Essential: Exceptional time management skills and ability to work unsupervised in the community.
Essential: Current Victorian driver’s licence
Essential: Ability to provide person centred care in the home environment for people with complex needs.
