Position Summary
The Transition Care Program (TCP) is seeking an experienced, innovative and flexible Occupational Therapist to work as a member of the TCP team.

The program is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long term care arrangements.

Essential Criteria
The successful applicant will need to be flexible in terms of working across both residential and community areas. The Occupational Therapist will need to demonstrate sound clinical assessment skills, and to have an understanding of the issues affecting patients with complex functional and cognitive needs and an interest in aged care issues.

The successful applicant will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team.

Selection Criteria Desirable: Experience in Aged Care
Essential: Current Victorian Drivers License
Essential: Current registration with the Occupational Therapy Board of Australia (via AHPRA)
Essential: Member, or eligible for membership of OT Association of Austraila
Essential: Demonstrated experience as an Occupational Therapist
Essential: Capacity to supervise students, Grade 1 staff, Allied Health Assistants, and promote professional competence and foster individual staff development
