Position Summary
The Outreach Service is a HACC and Commonwealth funded program providing service across the beautiful Frankston and Mornington Peninsula Local Government Areas. The team is integrated across Community Health Centres on the Peninsula.

The Occupational Therapist assists eligible clients by providing comprehensive home assessment; specialist recommendations regarding assistive equipment and home modifications; holistic client/carer support and education; episodic care co-ordination and referral.

The Outreach Service provides a supportive and dynamic team environment with an emphasis on innovation. We are seeking a highly motivated and experienced clinician with both full time and part time hours available.

We are looking for :
• Bachelor of Applied Science (Occupational Therapy)
• Eligibility for membership of Occupational Therapy Association of Australia.
• Registration with the Australian Health Professional Registration Board (AHPRA
• Experience and understanding of working as part of a multi-disciplinary team.
• Excellent verbal and written communication and report writing skills.
• Well developed consultation, problem solving, negotiation, and conflict resolution and facilitation skills.
• Current registration as a prescriber with the State Wide Equipment Program (SWEP), preferably with some Amber and Red credentials.
• Sound knowledge of current Occupational Therapy theory and practice.
• Experience with home visiting and environmental modification.
• Understanding of client self- management principles and the Commonwealth Reablement Model and Victorian Active Service Model.
• An understanding of Peninsula Health Community Health principles and values.
• Exceptional time management skills and ability to work unsupervised in the community.
• Commitment to ongoing professional development and continuous improvement.
• Current Victorian driver’s licence

Ideally you will have experience in assessing and managing clients with complex needs, a high level prescriber status with SWEP and experience in working with the Australian Building Standards and providing technical drawings.
