What You Will Be Doing

We are looking for a highly experienced Care Manager to join our Home Care Package Program.

The successful candidate will join a committed Home Care Package team working together to deliver person centered care to a range of clients from diverse backgrounds.

You will provide assessment, care planning, service coordination, advocacy and support to recipients utilising a consumer directed approach of the Commonwealth funded Home Care Package Program.

Key Responsibilities include
• Comprehensive assessment and ongoing Care Management
• Liaise and work in collaboration with service providers to broker services on behalf of consumers
• Connect consumers into health and social welfare services
• Actively empower and guide consumers with health and social education to improve and manage their health and well being
• Actively participate in service coordination and financial management of package funds and billing of services
• Ensure Home Care Package spending remains within client budgets and is compliant with Commonwealth Government guidelines.

What You Will Need

We anticipate you will have:
• Nursing, Allied Health or other relevant Health or Welfare qualifications
• Current AHPRA (Australian Health Practitioners Regulation Authority) registration or eligibility with relevant professional body.
• Demonstrated experience in care management with consumers of complex and diverse health and social needs
• Well-developed consultation, problem solving, negotiation, coordination and facilitation skills
• Exceptional time management skills and ability to work autonomously in the community
• IT knowledge and skills including efficient data entry and budget management
• A current Driver’s Licence and willingness to transport consumers as necessary
• Knowledge of Aged Care Reforms and the Commonwealth Home Care Package Program

Desirable: Experience working in community health and a strong commitment to the Social Model of health and consumer directed care
