Grade 2 and Grade 3 positions avaliable until April 2022.
Fulltime, part time or casual contracts avaliable.
Shifts avaliable from 7am - 10.00pm - 7 days a week.

Funding has been made available across twelve (12) hospital sites located within the South East, North East and West metropolitan health service partnerships (HSPs) to establish the COVID Care Navigator model.

What You Will Be Doing

Working in partnership with COVID Positive (C+) clinical teams including medical, nursing and Allied Health the Peninsula Health COVID Care Navigator provides a dedicated resource to:

• Support effective and timely discharge of C+ patients (assessed as being of low to medium severity) directly from the Emergency department or inpatients wards,
• Provide linkage with appropriate community COVID Positive Pathway Programs (as identified by the treating clinical teams)
• Monitor patient progress and recovery post-discharge to reduce unnecessary patient re-presentation
• Establish relationships with operational and clinical leads of the overarching Health Service Partnership (HSP), to ensure a standardised and consistent approach to the C+ patient care at a systems level and may be required to liaise with the Department of Health on these processes
• Work closely with internal partners (Allied Health, Care Coordination programs, Hospital in the Home programs, Hospital Admissions Risk Programs and other bed substitution programs) to identify and streamline the C+ patient’s care journey
• Coordinate the admission of a COVID positive patient who has deteriorated at home,
• Act as an advocate for C+ patients and their carers/families
• Provide clinical expertise to other staff specific to C+ discharge planning and pathways

The Role

We are seeking Grade 2 and Grade 3 Allied Health professionals interested in casual, part time of fulltime hours across a 7 day rotating roster working from 7am - 10.30pm.

The COVID Care Navigator will be able to work independently whilst at the same time contributing strongly to their team and the broader multi-disciplinary COVID care team. They will also demonstrate a willingness to participate in the evaluation of the pilot program and any related research and quality activities
